What is the Process for obtaining a Connecting the Dots Report?

Questionnaire based information gathering (length and detail required will depend on individual need).
Telephone/online/in person meeting 1hr - 1.5hrs.
School observation.
Out of school observation, at our Creative Studio in Morley.
Written report emailed after interviews and observations are finished. Approximately 5 week process. This will have additional handouts, workbooks and information supplied at the same time.
Access to a follow-up telephone appointment within 3 months of receiving the report
